This includes insurance providers like Medicare Advantage group plans, State Medicare-Medicaid Long-Term Care programs, Accountable Care Organizations, Senior Living Facilities, Continuing Care Retirement Communities, employers offering subsidized health insurance benefits, and various other entities concerned with the well-being of aging individuals.

While HealthyLifetime’s program is beneficial for individuals aged 30 and above, it particularly caters to those aged 50 and older. However, it’s essential to note that adults between the ages of 30 and 50 experience the fastest-growing increase in conditions such as high blood pressure, high cholesterol, and diabetes.

Our founders, distinguished Nurse Researchers and Scientists from the prestigious University of Michigan School of Nursing, bring unparalleled knowledge and expertise to the development and direction of our innovative healthy aging program. With research interests spanning various domains such as aging population studies, family caregiver science, and advanced nursing practice, our executive nurse leaders are widely recognized for their contributions. They are prolific authors in both national and international scientific journals, sought-after speakers, and esteemed advisors for expert panels, curriculum development, and consulting services in the medical and nursing fields. With over 30 years of research, we’ve developed a nurse coaching program tailored for independent living. Our specially trained nurse coaches deliver the HealthyLifetime program with a focus on long-term health and independence. We prioritize personalized approaches, allowing individuals to set self-selected goals and empowering them through learning. Our close collaboration with participants ensures better health outcomes, emphasizing long-term cost reduction and individual independence. Unlike other programs, HealthyLifetime intervenes early in the aging process to maximize the benefits of behavior changes, mitigate functional decline, and prevent chronic conditions from worsening.

Operations and Program Execution

HealthyLifetime is delivered entirely online, right to a person’s home on their computer, laptop,  tablet, or smartphone over the course of 8 weeks. Participants are paired with a Nurse Coach trained in the methods of HealthyLifetime. The Nurse Health Coaches help participants gain insights into their preferences, patterns, motivations, and barriers. Our program equips participants with the knowledge and skills necessary for long-term independence, fostering self-efficacy and resilience along the way. We empower individuals to make meaningful changes and lead fulfilling lives through personalized support and tailored resources. 

The program is an 8-week program consisting of two initial 60-90 minute assessment and action planning sessions, followed by six 30-minute motivational coaching sessions.  Nurses need an additional 15 minutes of indirect time for preparation and recording nurse notes per client.  We recommend 1.5 hours per week of group skill-building time and 1 hour per month of individual mentoring.

The maximum number of people that a nurse at 1.0 FTE can support at any given time is 30-35.

You will need to have nurses willing to be trained in the HealthyLifetime™ methods/strategies.  The online training lasts for five weeks and requires an average of 12 hours per week.  The training is mostly asynchronous but requires one synchronous 1.5-hour Zoom training per week.  You will need to decide who your organization will be offering the HealthyLifetime™ program to, how you will recruit participants, and enroll those participants. HealthyLifetime™ provides you with program surveys, access to the @gethealthie coaching platform, and a marketing toolkit to encourage program sign-ups.

The HealthyLifetime™ Program has been tested in one randomized control trial and two cohort studies by the University of Michigan School of Nursing for the Centers for Medicare/Medicaid and the Michigan Department of Health and Human Services.

The program option and training structure chosen by your organization may require adjustments to your nurses’ workflow to accommodate the training and onboarding process for the HealthyLifetime™ Program.

Because the program and training are customized to meet your organization’s specific needs, pricing is tailored accordingly. You can request a custom quote after discussing your requirements with the HealthyLifetime team. Additionally, we offer reimbursement options through CPT/HCPCS billing codes and various funding options.

The cost of training is determined by the unique training program tailored for each organization. As such, pricing is customized to align with the specific needs and requirements of each organization’s training program.

The training is provided by our highly-skilled, practiced and knowledgeable HealthyLifetime™ nurse health and wellness coaches with extensive experience in our strategies and methods and in the HealthyLifetime™ program.

Who the Program is For

This program is generally appropriate for anyone 30 or older. Adults 30-50 years old have the fastest-growing increase in high blood pressure, high cholesterol and diabetes, of any age group, making them an important high risk group. And those 50+ are more likely to start to experience declines due to aging. Both age groups would benefit from learning how to live healthier, longer, and thus, more independently.

This program intervenes early in the aging process when people have their best chance for long term benefits of changing their health behaviors, to live healthier, longer, more independently.

Our program is centered on promoting independent living rather than focusing solely on chronic illness management. While we do address strategies for managing chronic conditions, our approach is distinct in its emphasis on empowering individuals to self-manage their health comprehensively, leading to longer, healthier, and more independent lives.

Cost for an individual depends on the organization that you belong to. Every health plan, living facility or nurse counseling organization will have different co-pays (if any).

Program Details

You will be assigned a specially-trained nurse health coach who will work with you to set health and personal goals; help identify steps to meet those goals; coach you toward meeting those goals; and help track your progress so you can live a healthier, more independent life as you get older. The first two sessions are 60-90 minutes, with the next six weekly coaching sessions 30 minutes in length. Additional sessions will be available at an extra cost.

You will be asked to have weekly coaching sessions with your nurse. You will also be given tools to track your progress during and after the program.

No, the entire program is delivered through HIPAA-compliant video coaching sessions with your nurse right in your home on your own computer, laptop, tablet, or smartphone.

Yes, a computer, laptop, smartphone or tablet with video is required.

Yes, you will need an internet connection that can support a video visit through a Zoom call. Using a hotspot to connect to will not work. You also need to access the program on the internet in private, not a shared public place.

60-90 minutes for the first one to two assessment and planning sessions and then 30 minutes for the following 6 weekly coaching sessions.

You can stop at any time but your program fees may not be refundable depending on when you decide to stop.
